To redeem your rebate according to the product chart and amounts above, please ensure that:

• You placed your order for Covad service between March 4, 2003 and June 30, 2003.
• You placed your order for TeleXtend service between March 4, 2003 and May 31, 2003
• Your line has already been installed.
• You have your Covad order number. You will need that information to complete this rebate form. If you do not know your Covad order number, please contact your ISP for information

Step 1: Verify your eligibility by pre-qualifying online.
Step 2: Complete and submit your barcoded form before August 15, 2003.
Step 3: After submitting, you may check your rebate status online.

Rules and Regulations:
To qualify End Users must order Covad service through "Participating Reseller Partners" and have Covad service installed through "Participating Reseller Partners" by August 15, 2003. End users receiving Covad TeleSurfer services through "Participating Reseller Partners" and end users with existing Covad service through any other Covad Partner or from Covad directly do not qualify for this offer. Orders for TeleSoho and TeleSpeed must be placed between March 4, 2003 and June 30, 2003 through "Participating Reseller Partners". Orders for TeleXtend must be placed between March 4, 2003 and May 31, 2003 through "Participating Reseller Partners". If End User requests a downgraded speed after installation and after a request for rebate has been submitted or processed, End User will be assessed by "Participating Reseller Partner" all applicable disconnection charges as dictated in the "Participating Reseller Partner" end-user agreement and may be subject to charges for the rebate. Once Covad service is installed, to receive rebate, End User must mail in the completed rebate form postmarked by August 15, 2003. Covad will not pay rebates on any rebate forms postmarked after August 15, 2003. Rebate check amount is $75 on TeleSoho services with self-installation. Rebate check amount is $200 on TeleSoho services with professional installation. Rebate check amount is $200 on all TeleSpeed services. Rebate check amount is $750 on all TeleXtend services. Checks will be issued in the Company Name or End User name (if there is no Company name) on the service order and mailed to the address where the Covad service was installed. End User information is completely private and will not be transferred or sold by Covad. Limit one request per installation. Duplicate requests will not be honored. All submissions will become the property of Covad Communications Company. Offer valid for U.S. residents only. Offer good only in those states in the U.S.A. where it is not prohibited, taxed or restricted by law. Please allow 8-10 weeks after request is received and verified for delivery of the rebate check. This offer may not to be used in conjunction with any other promotion. Covad reserves the right to reject any rebate form that it deems, at its sole and absolute discretion, to be fraudulent or not to be in the proper form.
