Q. How do I find my Circuit ID number?
A. Your Circuit ID number is available from the SMART Account Manager at www.covad.net. You will need your Covad User ID and password to log in.
Q. How do I find my Covad Order Number?
A. Your Covad Order Number is sent to you by email from Covad when you place your order. If you do not have your order number please contact your ISP for further assistance.
Q. How do I find my modem name (CPE) and model number?
A. The modem name and model number can be found on the bottom of the modem itself.
For example: "ZyXEL 643 ADSL Gateway".
Q. What are the dates of the offer?
A. The offer is valid beginning on November 1, 2007 through December 31, 2007. Orders must be placed within this time frame to qualify for rebate.
Q. Do I have to use the installation street address?
A. Yes, checks will only be issued to the End User and company named on the order and mailed to the address where the Covad service was installed.
Q. How do I check the status of my rebate online?
A. Log on to our web-site at www.web-rebates.com/covad/ you can enter your customer submission number (provided when you prequalify for rebate online) and track the real time status of your rebate.
Q. Is this offer good only in the U.S.?
A. Yes, the offer is valid for U.S. residents only. Additionally, the offer good only in those states in the U.S.A. where it is not prohibited, taxed or restricted by law.
