  3. MAIL all required items in one envelope to the address listed on the form.
Step 1: Enter your serial number
To see if you qualify for this rebate, enter the 12-digit serial number of your Notebook PC in the space to the right and press the “Check Serial Number” button. Please make certain that you enter the numbers and letters 0/O, 1/I, and 2/Z correctly or your serial numbers may not qualify you for a rebate.
